Search engine optimization for cosmetic surgeons

I have been resaerching SEO and looking at a number of companies. My question is this.. how does one go about comparing the various companies? It's not easy to differentiat amongst them, and for marketing my practice, I've already been burned before. Needless to say, I'm not a big fan of search engine optimization people. But, I'd like to be proven wrong.
